Schengen visa(for short stay) application form questions?

Hi,
My husband and I are travelling to Europe.He is a citizen and I am a permanent resident of U.S.A,so only I need the visa.Questions regarding the form?

4.Date of birth- Read in one of the links that the date should be written like this (DDMMYYYY) but the form says Day-Month-Year.Is it okay to use hyphens?

7.Current Nationality - The same link says just write country code.Or can I write the spelling of the whole country?

11.What is the National Identity number?

20.Employer and employer's address and tel:
Only my husband works.So do I need to write his company info or say NA?Asking as I am going to send a letter from his employer.

22 and 23. We are going to Austria-Switzerland-France-Luxembourg.Entering Austria first but staying the longest in Switzerland. So 22.Austria-Switzerland-France-Luxembourg and 23.Austria right?

28.Entry permit for final country of destination?Is that applicable?

31.We are staying in 7 different hotels.Do I need to write down all the names,addresses and telephone numbers?
Also read that I need to write the address like this -country code-town-street-number, e.g. "USA-Miami-North Str.-8".Is this true?

33.My husband is paying for our trip.Should I mark "by applicant herself" or mark 'other' and say "Spouse"?

34 and 35.Are not valid right?

Sorry I have so many questions.But running out of time.Have to submit the application in about 12 hours.

Thanks!National Identity Number-I wrote N.A.
Never struck any of us.Asked some friends too.
Didn't write the SSN number.

I am really tensed now as I read that they don't process the visa if it's
not complete.I sent out the form on Saturday.

Please tell me what happens now??

What is meant by "visa being incomplete" ... providing DOB in MMDDYYYY format instead of in MM-DD-YYYY format is not the case. If you failed to completely provide your date of birth, then it would be incomplete, but as long as the information is readable, you are fine. It really also depends on the mood of whoever gets your application on the table. How did you send out the form? Online or mail? Which embassy of which country? And you are a national of...?

Hi Danisara,
The only thing I didn't write was the 'National Identity number'.
Instead of my SSN number I wrote 'Not Applicable'.
Sent my application through mail to the Swiss Consulate.
I am a national of India.

Anyway, it's too late to brag about it, perhaps they won't notice. Smiley Do you have any national ids in India? Maybe if you do not have any in India, your answer would be true too.

That's true.We don't have National ID numbers in India.

Hi Danisara,
I am back.You can guess why!
I got my visa.I am so relieved.
It reached the consulate on Tuesday and they sent it out on Wednesday which was super fast!!!

It is great, congratulations! You have got the visa in about a week if I count correctly, it was really fast. Would you mind sharing with other readers the location of the embassy? It is good to know which embassies do good job. Thanks.

Actually it was 2 days.I opted for the 2 day service through Fedex.
I applied to the Swiss consulate by mail in Atlanta.
I think it was also quick cos of the season.
